We
at Summit Acres Home Care are committed to providing quality
health care to our community and surrounding areas. We help
people of all ages receive needed medical treatment in the
comfort of their own home. Our goal is to keep families together
while allowing them to maintain their privacy, independence,
control, and peace of mind.
All services needed are discussed with the client's physician
and a plan of care is then implemented. All client care is
approved and monitored by a registered nurse and physician.
Skilled Nursing
Depending on the client's needs, our nurses perform many
services including assessing the client's condition, performing
dressing changes, giving injections, teaching clients and/or
caregivers pertinent health care information.
Home Health Aides
Home health aides assist clients with personal care needs and
homemaker services. They also provide emotional support and a
caring helpful hand.
Therapy Services
We provide a range of therapy services including Physical,
Occupational, and Speech Therapy. We provide IV therapy, pain
management, tube feedings, in-home x-rays and ECG's. Along with
other services that were once only available in hospitals and
physician's offices.
How To Reach Us
Our services may be obtained through a referral from a
physician, family member, and a hospital staff member preparing
for a client's discharge, or by calling our office at (740)
732-5712. Summit Acres Home Care is certified by the Social
Security Administration for participation in Medicare, Medicaid,
and Passport services. We also provide services through many
insurance companies and offer private payment plans. We serve
Noble • Guernsey Belmont • Washington Monroe
Counties.
To Qualify
To qualify for home health services under Medicare, you must be
in need of a skilled nursing or therapy service and must be
considered homebound. Under Medicaid, you must be in need of a
skilled nursing or therapy service and meet Medicaid guidelines.
Passport is a waiver program provided through Medicaid for
persons age 60 and over who are in need of services ranging from
continuous care to intermittent services. Please call if you
